    Program Manager – MDE


    (Part-time, 20 hours)


    Generations Family Health Center, Inc. is a regional community health center covering a thirty seven town rural region in eastern Connecticut with sites located in Willimantic, Danielson, Norwich and Putnam.  Our dedicated staff provides a full range of preventive and primary care medical services to all ages including health maintenance of chronic diseases, 24 hour on call, and patient case management, as well as complete behavioral health, and dental services at our Willimantic and Putnam site through the ACROSS THE SMILES mobile dental program operating in the Northeast service area. 


    Location: Willimantic, CT



    The (MDE) Program Manager is responsible for the supervision of all MDE appointments made at GFHC and the subsequent reporting. In coordination with the BH Operations Director, the Program Manager ensures tracking and receipt of all intake information and forms from DCF for MDE referral and coordination of all MDE exams at GFHC. The Program Manager will also ensure the timely and efficient preparation of materials for exams; collaboration among the relevant providers for completion of the exam summary, and timely transmittal of exam information to DCF. These activities include supervision of staff, DCF and provider relations, patient customer service, data entry, supply purchasing, and communications.


    Bachelor’s Degree with two (2) years’ experience in health care management or commensurate experience. Minimum one (1) year supervisory experience. Knowledge of public health issues, child development, and health care office practices. Knowledge of Medicaid regulations and HUSKY benefits.

    Requirements:  Must work as part of a team. Must have strong communication, interpersonal and supervisory skills.  Must be detail-oriented and able to carry out duties with minimal supervision. Computer proficiency desired. Familiarity with issues which effect children at risk and in foster care. Experience with medical records and MIS.  Ability to work within a culturally diverse work environment. Must have a valid driver’s license.
