    Generations Family Health Center, Inc. is a regional community health center covering a 33 town rural region in eastern Connecticut with sites located in Willimantic, Danielson, Norwich and Putnam.  Our dedicated staff provides a full range of preventative and primary care medical services to all ages including health maintenance of chronic diseases, 24 hour on call, hospitalization coverage and patient case management, as well as complete behavioral health and dental services at our Willimantic and Putnam sites and through our School Based Health Center and ACROSS THE SMILES, a mobile dental program operating in the Northeast and Windham service area.

    Job Summary: The home visitor is responsible for initiating and maintaining regular and long-term contact/support with families primarily through home visitation.  The home visitor may be required to work several evenings per week and occasional weekend hours.


    Coordination of Care:

    • Provides regular (at least weekly) and long term (up to five years) contact/support with families
    • Provides home visits to families in a timely manner
    • Provides family-centered and strength-based interventions
    • Establishes trusting rapport with families
    • Educates patients about health-related issues, pregnancy, preventive health maintenance, etc.
    • Educates patients on managed care and plans in which the health center participates
    • Makes and schedules referrals as per protocol
    • Assists families to develop goals and plans to accomplish these goals as per protocol
    • Assesses normal growth and development of children
    • Contacts patients as instructed regarding missed appointments, etc.
    • Utilizes resources to accommodate cultural needs of patients and customers such as dietary, religious, and language needs

    Knowledge and Skills:

    • Knowledge of human services in the area
    • Follows policies and procedures of Generations Family Health Center, Inc.
    • Knowledge of normal child growth and development and parent-child relationships
    • Maintains good communication with the public via telephones, letters, presentations, meetings, or events
    • Maintains necessary tracking systems to determine services delivered to families
    • Consistently seeks ongoing education and training to maintain awareness of how best to provide appropriate care based on age-specific needs

    Administrative Functions:

    • Maintains an adequate supply of forms and educational materials
    • Regularly attends statewide program meetings
    • Prepares required programmatic reports, logs, etc.
    • Reports requested information to comply with funding reports or collaborative relationships with other agencies or institutions
    • Documents home visits, screenings and contacts with families using Visit Tracker software system.
    • Demonstrates use of office equipment (i.e., copier, fax, e-mail, etc.)
    • Has the ability to lift up to twenty (20) pounds


    Associate or Bachelors of Arts or Science in human services or related field or equivalent in related work or life experience desired. Experience with case management/home visiting experience preferred. Bilingual (Spanish/English) candidate preferred based on patient population.
