Generations Family Health Center, Inc. is a regional community health center covering a thirty seven town rural region in eastern Connecticut with sites located in Willimantic, Danielson, Norwich and Putnam.  Our dedicated staff provides a full range of preventative and primary care medical services to all ages including health maintenance of chronic diseases, 24 hour on call, hospitalization coverage and patient case management, as well as complete behavioral health and dental services at our Willimantic and Putnam sites and through our School Based Health Center and ACROSS THE SMILES mobile dental program operating in the Northeast and Windham service area.

 

Location: Willimantic, CT  (Part-time - 24 hours)

Responsibilities

Job Summary:   The intake clinician is responsible for the coordination process of all referrals received.  The intake clinician will provide client centered behavioral health phone triage, intake/assessment, scheduling and related services to individuals and families who are seeking assistance for behavioral health outpatient services. Bilingual (Spanish/English) candidate highly preferred.

 

Essential Duties and Responsibilities:

  • Responsible for the direct coordination of all referrals for clients in need of Behavioral Health services (receiving, tracking status, scheduling based on clinicians availability, performing assessments as required, and referring out if necessary)
  • Triage behavioral health services phone calls and schedule in accordance to level of severity
  • Assumes the day-to-day responsibility for the entire intake process and procedures
  • Maintains an up to date database of all clients referred
  • Ensures  follow up with all referral sources and proper documentation in the medical records
  • Ability to understand all insurance protocols (Medicaid, Medicare, Private Insurance)
  • Participate in clinical team meetings
  • All other duties as assigned

Qualifications

Qualifications:   LMSW, LCSW, LMFT, LPC or any licensed eligible degree is preferred. Minimum of two years experience in the assessment, treatment of clients with mental health, substance abuse, psychiatric disorders, and associated conditions. Excellent communication skills, both verbal and written required. Excellent assessment, diagnostic skills and organization skills. Knowledge of mental health, bio-psycho-social influences and intervention, and developmental disabilities as they present themselves throughout the lifespan.
